Special Investigator's Statement

Investigation into the incident at the Beaconsfield Gold Mine on 25 April 2006

An independent Special Investigator, Mr Greg Melick AM RFD S.C., has been appointed to conduct a full and proper investigation of the cause of the incident at the Beaconsfield Gold Mine on 25 April 2006 which resulted in the death of Larry Paul Knight and the trapping of Brant George Webb and Todd Andrew Russell.

The investigation will consider and report on:

  • The cause of the event;
  • The adequacy of processes and procedures established by owners and operators of the mine and Workplace Standards Tasmania to ensure that the obligations imposed by Workplace Health and Safety Act 1995 were adhered to;
  • The steps that should be taken by the owners and operators of the mine to ensure that the health and safety requirements of the Workplace Health and Safety Act 1995, including measures to prevent or lessen the risk of a repetition of the event or similar events, are maintained at the mine; and
  • Matters incidental thereto.

Submissions are now invited from individuals or organisations regarding any matters relevant to the investigation. Any submissions could be made public, and will inform the Coronial Inquest into the cause of the death of Larry Knight, and if necessary be considered by the Director of Public Prosecutions.
